Injured in NZ? How ACC Physio Actually Works (No, You Don't Need a Referral)

New Zealand has one of the most generous injury cover systems in the world — and one of the most misunderstood. Every week, people delay treatment for days or weeks because they think they need to see a GP first, or they're not sure whether their injury "counts", or they're worried about the cost.

Here's how it actually works, in plain English.

You Can Come Straight to a Physio

This surprises a lot of people. You do not need to see your GP first for most musculoskeletal injuries — a sprained ankle, a strained back, a netball knee, a shoulder injured lifting at work. You book a physio appointment, we complete the claim form (an ACC45) with you in the room, and ACC processes it. Seeing your GP first isn't wrong — it's just an extra step and an extra wait that most injuries don't need.

What ACC Covers (and What It Doesn't)

ACC covers personal injury caused by accident — and that word "accident" is doing the heavy lifting. It means a specific event: you rolled your ankle on Tuesday at training, you lifted a box and felt your back go, you slipped on the stairs. Sport, work, home, school — the location doesn't matter, and it covers everyone in New Zealand, including visitors.

What generally isn't covered is pain that crept up gradually with no event behind it — a knee that's been getting sorer over two months of running, or a shoulder that aches more each week. Those are still absolutely worth treating (often more so, because they don't resolve on their own), but they're usually treated privately rather than under ACC. Some gradual-process injuries caused by work can qualify — that's worth a conversation rather than an assumption.

What It Costs

ACC pays a contribution toward each physiotherapy treatment — but it has never covered the full cost of delivering modern care, which is why nearly every clinic in New Zealand charges a co-payment (surcharge) on top. The surcharge varies by clinic. What matters is that an ACC appointment costs you considerably less than a private one, and lodging the claim costs nothing.

What Happens at the First Visit

    Your first ACC appointment, step by step:

  • Tell us the story: what happened, when, and what it's stopping you from doing. The specific event matters for the claim, so try to remember the day and the mechanism.
  • Assessment: a full physical examination — movement, strength, special tests — to establish a diagnosis and a baseline we can measure progress against.
  • Claim lodged: we complete and submit the ACC45 electronically. You'll get a claim number, and ACC will confirm cover (usually within days).
  • Treatment starts immediately: you don't wait for claim approval to begin rehab — early management in the first days makes a measurable difference.
  • A plan, not just a session: you leave knowing the diagnosis, the plan, the expected timeframe, and the criteria we'll use to know you're genuinely right.

If You Need More Treatment Later

ACC approves an initial allocation of treatments with each claim. If your recovery needs more — common with bigger injuries — your physio applies to ACC for additional sessions with clinical justification. This is one of the places objective testing quietly earns its keep: an extension request backed by force-plate numbers showing exactly what deficit remains is a much stronger case than "still sore, please send more visits".

One last thing worth knowing: lodge claims promptly. A claim lodged the week of the injury is straightforward; one lodged ten months later invites questions. If you're hurt, get it assessed — even if you're hoping it will settle on its own.
